Before your first appointment, we need you to fill out two sets of paperwork and bring these to the appointment.  Please print our new patient registration form and then print our new patient history form.  The registration form includes demographic information, such as your telephone, EMail, insurance, etc.  Please fill these forms out. There are several documents about policies for you to read. Once completed, you can Fax forms to 501-537-7412 or bring them with you to your appointment.  An alternative is to fill out the registration forms on the portal where it says "New patients start here."The new patient history form is to let us know the problems you are having, past treatments, medical problems, etc.  It is not available to fill out on the portal.
